I have seen reloads that failed to fire the first time or two .... what we eventually figured out was the primer wasn't seated all the way .... the first time ( on a few the first couple of times ) that the primer absorbed some of the force by moving a bit deeper into the pocket .... once fully seated the shell would fire ....
